December 5 to February 20: Students' Health Exam at NTU Hospital 

*Please note that NTU Hospital will be closed from February 8 to 14 due to the Chinese Lunar New Year Holiday.

1. Log in to the Online Health Survey and complete it.

2. Download the NTU General Health Exam Form and go to the Hospital to do the Health Exam accordingly:

Master and Doctorate's Degree NTU General Health Exam Form 

• Please note that the NTU General Health Exam Form needs to be completed in Taiwan.
• Please download the NTU Health Exam Instructions below and follow the instructions to go to the appointed Hospital to do the Health Exam.
  You may go to NTU Hospital (Fee TWD 900) from December 5, 2023, to February 20, 2024.

3. Please upload the completed NTU General Health Exam Form in the Online Registration System during the Online Registration Period (from February 16 to 20).
